Program Guidelines
Our plan offers several benefits with no yearly maximums, deductibles, or claim forms. The initial activation fee is waived, and there are no limits on pre-existing conditions, no coverage denials, and no waiting periods.
Exclusions May Apply
The one-year plan is non-transferable and non-refundable, and it is applicable only for dental treatment at our office. It can only be used at Mountain View Pediatric Dentistry and is not valid at any other location. Please note that this plan does not cover sedation services.
